Do you manipulate the tools collection in code in any way within a customization on that form?

I dont see anything like I was mentioning. Is this a Runtime or Assembly dashboard? You can try to open the dashboard using the TEST APPLICATION button, and see if the problem still occurs. I am thinking it will have to be a compiled assembly for this feature to work. I have no basis for saying that, just a hunch
